Parents-
If you have a kindergartener who will be 5 by September 1, 2021- please fill out this form by clicking here
If you also have a neighbor or friend who will also be attending Liberty, please refer them to our website and have them fill out the form from that site or you can copy the link: https://forms.gle/f6JFxwz388eFtjDK9 and send it to them if it is easier.
We will invite all incoming parents to come into our school on Wednesday, April 21 at 1:30. At that time we will have be having an open house where you can drop off the required paperwork to start kindergarten.
We will need the following from you:
- Originial Birth Certificate
- 2 Utiliites or Special Permit if out of District
- Immunizations
At this time due to COVID, we will not be doing tours of our school. All kindergarten assessments will be held the first week of school and you will be contacted in August to set up a time for that assessment.
